Precision Treatment to Preserve Teeth & Nerves

Director Dr. Ohara

Why We Prioritize
Preventive Dentistry
& Precision Treatment
— A Message from the Director —

— Based on My Own Experience of Having 13 Cavities Treated —

I personally had as many as 13 cavities treated before entering dental school.

Childhood photo

I loved soda, chocolate, and sweets. I often drank sports drinks for hydration.

My family did not run a dental clinic, and at that time I had no knowledge of preventive dentistry. Whenever my teeth hurt, I simply went to the dentist to have them drilled and filled—this cycle repeated for years.

When I entered dental school, I was shocked to find that many of my classmates—especially those whose parents were dentists—had never had a cavity in their life.

My life after entering university

After learning “how to avoid cavities” at university and putting it into practice, I have not developed a single new cavity for over 15 years.

This experience inspired me to start sharing information online to help people understand how to prevent cavities in the first place.

SNS activities

Things That “Prevention Alone” Cannot Protect

However, I also had another major concern: the deterioration of old fillings, which required retreatment.

Condition of old fillings

Even if no new cavities form, bacteria can infiltrate through gaps in old fillings or crowns, causing recurrent decay inside. With every retreatment, the restoration becomes larger.

Through this personal experience, I came to believe that “thorough preventive care” and “precision treatment that minimizes the need for retreatment” must work together like two wheels of a cart to protect patients’ teeth throughout their lives.

Precision dentistry concept

Japan’s universal health insurance system allows everyone to receive dental treatment at low cost. However, this affordability often leads to a lower awareness of prevention.

Preventive dentistry awareness

Without strong preventive habits, no matter how good the treatment is, the teeth will eventually develop new issues and need additional work.
That is why our philosophy is: first, provide thorough prevention, and then provide precision treatment.

As a result of this philosophy, we perform implant treatment only about once a month—sometimes not even that.

This is proof of our commitment to “preserving natural teeth.”

Principle 1: Preventive Dentistry (99% of Oral Health Depends on Daily Self-Care)

Self-care importance

Professional care at the dental clinic occurs only a few times a year. 99% of the time, your oral health depends on your own daily self-care.

Self-care tools

Of course, we provide thorough plaque removal using AirFlow and apply high-concentration fluoride (9000 ppm) to create an environment that resists cavities and gum disease.

But above all, we focus on self-care instruction.

Self-care guidance

We teach how to choose the right fluoride toothpaste, how much to use, how to rinse after brushing, and how to use interdental brushes and floss properly.
Providing patients with the “knowledge” and “skills” to maintain oral health at home is one of our most important missions.

Principle 2: Rubber Dam Isolation (Minimizing the Risk of Retreatment)

When placing fillings or crowns, we always use a rubber dam (a protective sheet placed in the mouth).

Rubber dam isolation

Even the slightest contamination of saliva or bacteria on the prepared tooth can cause the bonding to fail, creating invisible gaps.

During root canal treatment, contamination can allow bacteria into the root canal, causing future infections.
To prevent recurrent decay or root tip disease caused by bonding failure or debris, the rubber dam is absolutely essential.

Principle 3: Operating Microscopes (Used by All Dentists and Hygienists)

Improving the Accuracy of Treatment
With the tooth isolated by a rubber dam, we use an operating microscope to work under magnification many times greater than the naked eye.

Operating microscope

We check carefully for any remaining decay and ensure that fillings and crowns fit with micron-level precision. This “accuracy check” is key to preventing retreatment.

Transforming Patient Awareness
The greatest value of the microscope is not only improving precision— it is allowing patients to “see the reality with their own eyes.”

Our hygienists also use microscopes during cleanings and periodontal care, recording the procedures and showing the videos to patients.

  • “I didn’t realize my teeth were this dirty!”
  • “I had no idea my cavity was this big.”

The impact and surprise patients feel often speak louder than words. Their mindset shifts from passively “getting treated” to actively thinking “I need to change my daily habits.”

This motivation is the first step toward preventive dentistry, and one of the greatest benefits of using the operating microscope.

5 Key Reasons to Choose Kuribayashi Dental Clinic

Microscope Treatment

① Precision Treatment & Prevention — Preserving Teeth and Nerves. Microscopes Installed in All Treatment Rooms.

Not only our dentists but also our dental hygienists perform treatment using microscopes. With enhanced visibility, we minimize unnecessary drilling, avoid removing nerves, and reduce the need for extractions. Over 200 patients visit us monthly, yet we perform dentures only about once a year and implants once a month or less. Our photo and video explanations also help patients clearly understand their treatment.

[Rubber Dam Direct Bonding Treatment Details]

  • Cost: ¥50,000 + tax
  • Treatment Visits: 1
  • A minimally invasive treatment where tooth-colored resin is directly applied to restore shape and function. Using a rubber dam and advanced bonding materials allows long-lasting, natural-looking results.
  • Risks / Side Effects:
    • Deep cavities may require nerve treatment.
    • Can chip or detach depending on bite force.
    • Possible discoloration over time.
Direct Bonding Case

[Rubber Dam e.max Tabletop Veneer — Treatment Details]

e.max provides strong bonding and high durability. With modern adhesives, tooth reduction can be minimal, preserving tooth structure.

  • Cost: ¥140,000 + tax (e.max tabletop veneer / zirconia crown)
  • Treatment Visits: 2
  • Risks / Side Effects:
    • Deep cavities may require nerve treatment.
    • Chipping or detachment due to strong bite force.
    • Marginal discoloration over time.
e.max Veneer Case

[Rubber Dam Shield Restoration Technique]

  • Cost: ¥55,000 + tax
  • Treatment Visits: 1
  • A method used for cavities extremely close to the nerve. Instead of removing all infected dentin, the area is sealed using strong adhesive materials to prevent exposure and protect the nerve.
  • Risks / Side Effects:
    • Nerve treatment may still become necessary.
    • Possible chipping / detachment.
    • Discoloration over time.
Shield Restoration Case

[Rubber Dam Partial Pulpotomy]

  • Cost: ¥77,000 + tax
  • Treatment Visits: 1 + follow-up
  • Appropriate for deep cavities or early pulpitis. Infected upper pulp is removed while preserving the healthy nerve inside the root. MTA cement is placed, followed by a crown.
  • Risks / Side Effects:
    • Temporary pain may occur.
    • Nerve may die, requiring root canal treatment.

② Comprehensive Preventive Dentistry — A “Dedicated Dental Hygienist System” to Prevent Oral Disease.

Many people visit the dentist only after symptoms appear, such as tooth pain or bleeding gums. However, cavities and periodontal disease are irreversible once tissue is lost. This is why our clinic focuses on prevention, not just treatment. To maintain your lifelong oral health, a dedicated hygienist thoroughly supports your care.

Why do we insist on assigning each patient a dedicated hygienist?

  1. 1. Early Detection Through Continuity
Seeing the same hygienist each visit allows even subtle changes—gum color, tooth surface, brushing habits—to be noticed early.

  2. 2. Personalized Prevention Plans
Your oral condition, saliva quality, lifestyle, and health background are all understood. This enables tailored, evidence-based prevention—not generic advice.

  3. 3. A Relationship That Encourages Communication
Patients feel more comfortable asking questions or voicing concerns when trust is established. We value communication to help you achieve the best oral health outcomes.

  4. 4. Motivation Through Partnership
Prevention requires both professional care at the clinic and self-care at home. Your hygienist supports and encourages you to maintain healthy habits with confidence.

Director

Yuta Ohara

【Preventive Dentistry × Precision Treatment】

I am Dr. Ohara from Kuribayashi Dental Clinic Marunouchi.

It is said that the same tooth can only be treated 4–5 times in a lifetime.
Silver fillings last on average about 5–10 years. If your first treatment is done in your 20s, by your 50s you may already have undergone more than five treatments on the same tooth, significantly increasing the risk of tooth loss.
In fact, the average number of remaining teeth in Japan decreases starting in the 50s, and from there, people tend to lose one tooth every 3–4 years.

We are committed to preventing dental problems before they occur and providing precise treatments that minimize the recurrence of decay.
If you wish to keep your natural teeth for life, please feel free to consult us. We look forward to welcoming you.
